What causes "Weird signal from device" error?

I'm using a PoKeys57E with cnc4pc M16 board. Machine is a lathe. It works fine when it works but I often get this annoying error. Once I see it the machine then stops responding and then a "Connection with the device is lost" error comes promptly. The only thing left is to power-off/power-on 57E and restart Mach4.

I have checked all the wiring to make sure if I have a grounding loop but everything looks fine.

When I first get the 57E from cnc4pc it had v4.1.xx firmware installed. I then updated it to PoKeys v4.2.39 but nothing changed.

Anyway, what causes this error? Is there anything I can do/check?

By the way, I have tried the firmwares bellow but lathe encoder does not work with these firmwares so I stick with 4.2.39.

Finally found the problem. It was the 20 cm wall type cat5 extension cable I use in the electronics box. When I remove it and connect pokeys directly to the computer (with a standard cable) everything is fine. I can't believe myself I didn't try this at first.
