one limit switch triggers two green input port pins

I have three switches acting as homing switches for the three axis.  While the x and z switches appear to work fine, the y switch turns two yellow port pin light indicator in mach's diagnostic window.
I have checked the connections to the breakout board as well as the config screen to make sure that there are no double entries.  Everything checkout fine.
The problem is consistent, that is, it's as if one switch was tied to two port pins
Incidentally, the machine and the homing work fine.  I discovered this anomaly while trying to trace noise issues that occasionally stop the machine.

Any clues?

Load the standard screenset and see if the problem  is still there. Looking at the xml all seems fine and I am thinking  it may be that the LEDs on the screen have the same OEM.

Unless I misunderstood you, the file that I uploaded is the standard screen. I simply changed the file name due to a conflict with a similar file here on the forum.

It might have something to do with the breakout board.

Your xml says the screen you have loaded is  1024_offset.set the standard screen is 1024.set

Try swapping the connections on the BOB for two switches.  You may have a short or bad chip on the BOB.  If you swap the wires and the double-LEDs move to another axis, then the BOB is defective.
