OK so the program close confirmation has stopped occuring (e.g. when you clicked the red square with the white cross in it: top very left of program window). I think my lad may have ticked the box to say "Don't show again"

How do i get it back?

I thought I'd find something in one of the .ini files but that wasn't the case.. neither did I find anything in the registry.. so they've hidden this pretty well.. i'm going to try and simply reload Mach4 and see if that works.. I'll let you know.   

It’s located in Mach4GUI.ini, this file is in App Data>Roaming of your Windows user directory. Some other useful stuff in there as well for repositioning windows if they open off screen.
