Mach3 and G-Rex / Mach IV Tool Offset

I have my GREX/Mach IV system up and running.  Great stuff!    It appears that the start and end points of a line are wrong when using part offset and/or scaling.  Has anyone else seen this?

I saw it in the 'Mach2.6.11.pdf' file.  It is a screenshot of the engine configuration, ports and pins page.

I notice that on the engine configuration ports screen there is a section labeled "USB Mode".  Can anyone tell me what hardware is required to use this mode?
