I didn't say it's because jerk ,I said because there no jerk in Mach4 you must put very very poor acceleration ,then the cv become strange behavior

1.as i see even in mach 3you have some problem with yours shape( final position not same as first)
2. i had a lot of problems with cv with mach4 i can suggest you: try increase the acceleration /deceleration
    but i can tell you from start this solution its problem because mach not support  jerk and its big problem with increase acceleration
    but for us this the only way we solve all that cv problems with mach4

first because its seam its real his problem
but second ( not less important) from days steave left this forum ,real answer from mach hard to get
its much useful for you try save time and look for answer in place maybi you can get them

in any case if use mouse or keyboard my first suggest its not use wireless(i know now several will said we use...., and...)but as my experience
with more then 30 machines with
 mach4,wireless mouse and keyboard in some ceases give unexpected behavior

i must agree with Craig spechieli in such cycle like change tool
that involve movement ,output comand,input check...
and there is big rule you must put on yours mnd " the unexpected behavior always will happened"
you must think in every movement what can be fault , not only by machine but also with worker ,
remember very important  point ( for me it was big headache)if you press stop while m function run ,Mach will stop
only movement ,all other continue run
in other word if you press stop while change tool ,the machine will stop move ,but the spindle will open and tool will fall and beake
so this one sample  why you must think about every option in every point of time in yours cycle

I can tell what I do ,I don't know other
I create c# application that conect by rs585 to all devices on chain ( normal 6 servo pack and one inverter,) from servo we read the absolute encoder( it's stored on 7 buffet ,it's took me several howers understand how convert it but now it's work perfect
One big advantage it's that Mach it's not real time system but this absolute give me the options to compare in any time between what Mach think and what is real

It's real powerful ,but sure need alit of practice and tests
Actually who help me alot was steave and wish he will back one day
