poppabear made a plugin and screen set to manage IO. I don't have a link but if you search the forum you will find it. You don't have to use the screen set but it helps get things sorted out. Then you can use your custom screen with his plugin.

I've had mach on a lot of machines and never had that one pop up. I'm grasping at straws but maybe your "look ahead" settings?

If you are using a motion controller it may be a problem with that.

I've just changed to a UC100 and found the change over very easy and smooth apart from a little problem with the a axis not working at first.
I've also bought UCCNC to try but not had much chance yet and also most of my work involves using the wizards in Mach, stuff like thread milling and keyways is easy with the wizards. I guess I could write the same code by hand but I'm an occasional user so forget bits.
I'm not sure if I'd change to mach4 either, 3 seems to work for me at the moment and I'm not seeing any problems with CV on my mill.
However I'm thinking of changing over to UCCNC on my router as that is only used with CAM and goes much faster so I gess it'll take advantage of the better CV in UCCNC.

Is there any stand alone programs you can get that would replace the wizards in Mach?

Can't you run mach without a motion control selected and use the wizards in mach while UCCNC is open? Just do an edit save as from mach and save it to a directory where you can grab it from UCCNC.

I'm running UCCNC on 2 routers right now and I won't be switching back to mach at this point. The CV is just so much better.

can you switch wires around and see if it's hardware or software related. Run the a axis rom the X axis pins.

Well I'm moving over to UCCNC as I have 4 of his controllers. All the elements are in place for it to do everything I need it's just a matter of getting new macros written and some custom screen work. So far it seems to be pretty good but I have a long way to go before totally trust it. At least it's being developed by the company that makes the controller and I think that's an important thing.

Not to put too fine a point on it but I just finished changing out a tap and scrapping a part because the spindle failed to execute a M4 at the end of the tapping cycle. It just lifted until the tension compression holder hit bottom and snapped. Fortunately it was an 8/32 so it broke without any other damage. If it was the 5/16 it would have been ugly. Now it has been performing this code for the last week 1680 times and never missed a beat. It screwed up the week before though.

I've improved my odds by starting mach, shutting down mach and starting it again. I seem to crash a lot less this way.

CNCdrive has no plugin yet for mach4 (they are working on it) and in my mind switching to something that is still in development on both the program AND plugin from two different vendors is just setting yourself up for frustration.  It's the same problem I have now with Mach3 and the UC-100/300 motion controls. Is the problem with Mach or the plugin. We all know mach3 is riddled with random bugs so it's hard to hold Balazs feet to the fire to fix a plugin that may or may not be bad.

The same business that developed Mach3 is the same business developing Mach4. nuff said:)

I have the same problems as you. It will make a difference in the quality of the cut. It's really a problem if you are going into a corner that has the same radius  as the cutter. It can be quite violent. Any backlash at all will just exacerbate the problem. When I'm working on it I put a shot glass of water on the z head and watch the water vibrate and shake.
