if you are running Mach3 then check the version number. 
I think version # 62 works ok but not 66.

Function Cfgs at the top of the screen then Reset Device Sel...  Restart mach3 and it will ask again.

Did this a few years back Mach3 three spindles controlled by tool numbers on from the CAM program.  Simulated a Beese Rover machine.

This is a simple screen I made a few years back.  As you can see the operator didn't have much he could screw up.
No menus
No tabs
No other screens

On the "Settings" tab right upper corner input the "Rotational Radius" A as the radius of your part.
