I could just tell you but then you'll have another question, and another, and another...  Instead, why not carefully read the Mach3 manual and do your best to learn, and if you still can't work it out come back.  CNC is complicated, you have to learn for yourself.

Actually that's more than a breakout board, it's also a motion controller.   People on here have used them but often have problems.  To start with it needs a plug in for mach3 or mach4 - does the documentation say this is available with the unit?

Well, assuming that what you have is like the one in the photos with a 10k speed control potentiometer, then probably one end of the pot will have 10V on it and the other 0V.  The wiper will then give a variable voltage between 0 and 10v to control speed.  So wherever the wiper connects to on the PCB needs a variable voltage over this range. connected to it, which would presumably come from the C89 board.  One thing to look out for is that the motor controller probably doesn't have mains isolation so that would have to come from the C89 board which might support it - but as the documentation on the CNC4PC site is so useless I can't tell you.

Is the UC100 connected to a breakout board?  As far as I know (I have 2 of them) its outputs are all 5V, where are you measuring 9.7v and what is your circuit?

Mach 3 or 4?  On Mach 3 if you change from Radial to diameter you certainly need to restart.  No re-calibration should be needed (not quite sure what you mean by calibration) but you will need to re-home.

You need also to click the "Spindle" button on the manual or auto screens to tell Mach that it should be displaying speed.

The index signal from that Hall sensor needs to be connected into the index input on the interface card and provide one pulse per revolution of the spindle.  To generate that pulse you need a small magnet on the spindle and the sensor needs to be mounted quite close to it (probably a mm or so) to pick it up.  Unless you provide the index pulse you will not be able to either indicate spindle rpm or do threading.  So what is your sensor picking up?

The sensor photo you showed is a hall effect device.  You must have attached a magnet to the spindle for it to pick up - a photo of that would help.

Your spindle debounce is 1 x 40 us but the other debounce is set to 50  x 40 us which is 2 seconds!  Seems a bit long to me.
