Well said. You are not "the only person  in the world (at the moment) that wants to use [ E1P0/E1P1]".  I am having the same problem with my little
laser diode machine, and I, too, experienced the delay in trying to trigger the diode with axis movement.  My work around of using the C axis
direction pin is a less than satisfactory compromise, and forces me to use a custom gcode post processor and mods to my own software program.
Perhaps your post will lead to a proper fix.

I vote with Hood.  I've personally experienced the same problem, even with new anti-backlash nuts.  Tracked it down to a combination of machine flex and sticky axis bushings on my machine, but it can be any of several mechanical problems.  It is not a Mach3 problem.  Disconnect the axis drive and see if it moves smoothly end to end by hand.

Please let us know what you find.

Before you go to a lot of work and expense rewiring your limit switch circuit, try adding a small ceramic capacitor (.1uf/.01uf) between the limit switch input connection and ground on your controller/bob board.

I tried several times to post this to your "Laser" thread, but blocked at each attempt, hence this new thread.
 
Thanks to Dave, here's my version of his butterfly (http://www.machsupport.com/forum/index.php/topic,15111.0.html),
and many thanks are due to you as well for showing me the way in some of your previous posts and on your web site.

I burned it from 1/16 inch hobby basswood at 4 ipm using a 445nm laser diode with my own design constant current driver
set at about 1.1 amps.  I had to burn it from both sides to get clean parts release, but almost made it through from just
the one side.

With my setup, I am able to burn various woods at up to about 30 ipm, but I can not do gray scale pictures with Art's plug-in
(need more laser power for that).  After the first of the year, I may set up another 445 diode at max spec power to see if
it gives better results.

For other readers who may be thinking of trying a laser diode, I use the C axis direction pin with +0.0005 inch as laser off and -0.0005 on.
This greatly minimizes the "dots" at the start of burn lines.  I pre-focus the laser to table surface and use the Z axis to adjust
for different wood thickness.  It was a PITA trying to reset the lens frequently.

Since it is a USB printer, can't you just "pull the plug" on the USB cable to the printer unless printing?  Better safe than ruin a long job.  Windows frequently does strange, unexpected things.

I install from USB flash drives since my three machine computers do not connect to the WEB.  No problems.  Are you running the driver test after each new installation?
