This sounds like your velocity/acceleration settings may be too high on your Z axis.  Try reducing them until the G0 Z0 command doesn't cause lost steps.

I will assume that you are checking the Z bit being perpendicular to the table by measuring against the bit.  I think you may find that while the bit may be perpendicular to the table, the Z axis rails/rods are not.

Measure the Z rails and assure perpendicularity first, and then the tool bit/spindle.

You just may be expecting too much from the stepper at 150 ipm.  As the machine heats up, a very slight binding of anything from expansion may be more than the stepper can handle at that speed.  Slow it down to 100 ipm and try it.

While fine tuning the focus of a new lens for my laser diode the other day, I decided to test how fine a spot the lens was focusing too.  This is a dried pinto bean with the maximum height of the letters at .025 inches / .635 mm high.  I think I could have done even smaller engraving if I had reduced the laser power more, but I ran out of play time.  :D
