OK - as you haven't posted your code (except for your tiny snippet) I can't show you where the leadins are but using my crystal ball I see you set at least one of your straight leadins to be 1/10th of an inch long. Nope - sorry - I just can't see your arc leadins - must be losing my touch - or need a new crystal ball maybe... ;)

Hint: the ONE leadin you have shown us is line N850

You need to pass the X and Z values as parameters to your macro.
Your M178 Z3 X2 does not do this - it calls M178 with no parameters and executes Z3 X2 with whatever modal movement command (if any) is currently active.
See section 4.4.5 in the Customizing Mach2 document.

OK - tried 43.22 - no go, 42.40 - no go. The latest version that works is the one I started out with i.e. 42.20.

Hi Hood, on the machine in the vid I'm using 42.20. (never got round to updating it)

Coincidentally I have an eye on CV being "the problem". (or more to the point a MYTH about CV being the problem).

When Mihael asked me to post my xml I did a quick compare of mine and his. There's basically just one difference. I notice he has "Stop on CV angles > 89.9"

Mihael please turn this off (untick it in general config) and try your toolpath again.

You can do what you've described no problem at all in sheetcam. I'm afraid it's RTFM though because it's all in there.

Here's an example pic of a sheetcam setup where I've set up my table size, my stock size and nested 3 (.dxf) parts.

CV is plain "Mach out of the box" - lookahead 20, everything else off. System is an old AMD 1.5GHz / 512MB with onboard graphics and parallel port.
