OK, here are the input voltage levels for HC245

If Vih is on the higher side, and your parallel port does not have the 3.3V levels but a bit lower, then HC245 will have it's outputs "somewhwe" where it wants to be at the particular moment.

Yes, the motor reverses without a change in the direction voltage.

No. It is the voltage levels on parallel port.
According to the datasheet of HC245, your higj logic levels could be somewhere in undefined area for Vih.

After an E-stop or Reset, Mach can not guarantee the position of your machine. Specially as the lookahead buffer is emptied after one of these two actions.
If you want to keep position of the machine, you should first FeedHold the program and then Stop it.
So, after e-stop/reset you have to home your machine as the position is, for sure, lost.

Whenever you had coordinated motion which includes Z axis, your max speed would be 1200 mm/min, regardless of commanded 3500 mm/min. Coordinated motion is as fast as the slowest axis.

You can not cascade alarm outputs as per your drawing as your alarm input will only be active when ALL three alarm sources are active.
Consider output transistors as switches. For alarm input to be activated, all three "switches" has to be "closed". Either opened switch will render alarm inoperative.
