See section 3.6 in the write up. What you do is just have very shallow depth of cuts ( .0002" ), and instead of cutting the thread, the tool  will just scribe a lines around the piece.

Make sure your config is correct see figure 7.1.2

Can't really tell from the picture  because of the reflections and how the pic was taken ( not perpendicaular to the piece).
Looks like the center is different due to machine moves. I am thinking it is  a mechanical problem.
I would, for example,  do four  circles of say .020" diameter / four equaly spaced about the center. If your machine along with setup etc. is
 really good you can engrave those four circles say 5 times and see how well they all engrave. You can measure any
differences.Not bragging, but i probably couldn't see much differences under a microscope with mine other than spindle run out.

Just post code for the original and after the offset and look at the locations and you should see the difference in the gcode. I had the same problem zooming in.

- What rpm do you have the spindle set at? When you use the threading wizard for your code, did you click the "Calc the number of passes" button? When it clcs, it will tell if your axis settings have the capability to thread at the inputed rpm. If you get a warning then you need to lower the rpm or increase the accel and velocity of the axis.

- How stable is your rpm? The more stable the better.ie; one or two rpm varying ok
- Don't thread, but do the tests i talk about in the manual.If you can't scribe a single line then no use trying to actualy cut a thread. Just saves you time.
- make sure all configurations is correct

What went wrong on the sixth pass?

I am thiking your running at 1750 rpm and that is fast for threading ......

Hmm....
I don't remember any problems and have done some offsets with 1/32" / .032" cutters in fact even smaller with almost a point.
post your dxf file and maybe someone  have a look. No fixes to LazyCam as it is what it is. I remember the scaling somewhat and say you draw it 10x and then set the scale in Mach to .1, never had to use it though and i have done some small stuff.

The thumb and finger are just fine and no problems at all. It did take a long time to regain full thumb movement, say a year. Nothing was broke , it was just just pulled out of the joint, twisted and kind of flattened out from the glove squezzeing it. The finger nail just fell off. The finger nails came back but i remember always measuring it as it grew about 1/16" / month....so it took about 6 to 8 months to come back and even then another 6 months to look like a real one.
At that time i had to then fill out in triplicate and explain all to the company about my off the job injury, no lost time though,had to  become a safety rep,then serve on the safety committee, give safety presenatations,  and a whole bunch of other *********. Thank goodness it wasn't lost time as all the folks would not have recieved their 50 cent safety prize and I would have really been in the dumper. ;) Now days they just replace ya with a new human model of current vintage......  :D

Yep, all is well. Happened a long time ago.  The glove  was nailed to the shop wall as a reminder for years.
At least the boss, my wife, didn't fire me.......... ;D

Alton,
Get the code just for the letter O, but include any G0 or G01 before it.
You may want to try exact stop and constant velocity modes.
Like Russ, said reduce 50% as that should allow you headroom, but start there after
the retuning and work you way up still leaving a safety margin of say 30%.
May run slower, but at least you avoid possible problems. If you are using backlash compensation
reduce that also. Do as you wish since, i work on the conservative side on speed for the sake of movement reliability.

BTW, went to your site some very nice work there.
