If the index sensor is working and  appropriate for rpm range (  look at the specs of your sensor ) then the real time true rpm shown in the DRO  is more accurate than you will probably be able to measure  ( at least I know that for a fact and use the PP ).

The LED won't / can't  update fast enough to indicate each time the index is read.
Just consider it a visual that the index is working  since it's blinking randomly.
BUT
You will see the LED change from Waite for Trigger to Threading and Mode indication
from G94 to G95 via LED's every time when threading.

The problem is that the generic screen just sucks both functionally and  if your trying to monitor some things
amd that is why I use  a custom screen set.

Use Spindle Feedback In Sync Mode - required for lathe  threading as Mach will then use the index to change from G94 to G95 to sync for the multiple threading paths.
The time it takes to read and implement the change can be affected by index debounce and spindle speed averaging.

Spindle Speed Averaging - Used by Mach in threading and averages the rpm
 ( out to 4 decimal places ) over 8 revolutions ( from what I remember when testing ) during a path and uses the result for the the next path. There is more to this and would need find and look at more than 300 pages of notes. This is internal to Mach and you have no access to it. I would just check it and forget about it.

A simple test of the spindle sync would be to use a thin disc and see the scribed
mark made by the tool on the disc's cirmference. It's also a good test to see if ones lathe is capable of doing multiple start threading.  Same old remark, threading only as good as your lathe SYSTEM.

I get the same result on my end  per the first pic of inputs you posted.

The circle is a crop circle and to get rid of it untick Reversed Arcs in Front Tool Post
in configuration.

After you post code  in the wizard, exit back to the turn screen, zoom in  the graphics screen, and step through the gcode and watch the tool movements.

If all you want to do is put a radius on the end of the stock use a different wizard.

Note ....... "These wizards are donated by users ....., they are unsupported, but found to be usefull."   ;)

HMMM................................

I remember someone who retired and decided to go back to work to buy whatever he wanted for his joy.
So after working a little he bought a  big Harley bike all dressed out with every thing he could put on it. Not cheap!
While talking to the dude on the phone about the picture of the toy he bought I could hear his wife yapping at him about buying the bike, he said hold a second Rich, he yelled back to his better half and said:

"after working 40 years, 5 kids, and sacrificing for all ........ I'll now buy whatever I want and may I add that the bike
only has one seat and there is no room for your fat ask to sit on it!"

She never mentioned anything about that bike again.

They are still married!

Just a thought that came to mind..................  :D

There is no documentation on how to use the turn wizards.
Some work only in radius or diameter mode and not both modes.

I assume you are using the OD ARC WIZARD.

I'll post something quick and dirty for you on how to use.

Have  look at "Shumatec" for display kits. Our's are really old and still working.
I don't use mine anymore but a friend is still using his on his lathe after 10 years.
I'll see if  I can get more info.  Can do a three axis machine for around  $250.

Unfortunately I just don't provide help for the TB*********x controllers, any of that series,  since those Chinese controllers are just problematic.
Other word descriptions for them would not be appropriate to post. >:D
