Not sure where you got your demo version from but suggest you download Mach from "Downloads"  at the top
of the sites page. Here is a link.

Placing your lic into the folder where Mach was installed shoud have worked. You should get an English screen when it opens.
To change to an English screen, when in Mach click ---------- View>Load Screens> then select 1024.set ( should be a Mill File).

Glad you got it working and thanks for the comments. I doubt that LT will be updated and will probably remain as it currently is.
1) is known and oe will just need to manualy deal with it
2) Not sure about that one
3) Just need to manualy modify when you review the code

Can't recall any problem relative to code posting for both radius and dia mode.
Try again, make sure the DXF is drawn for dia, LT is in dia mode, and Mach config is set for dia mode; ie; keep the same mode from beginning to end of the process.

Not sure what your trying to do.
Lets make it simple so you have a base to start from and then you can play around with other settings of Mach  or LC and see the affects on code and the axis movements.

Set your tool at the top of the material and on the 0,0,0 of the part. No work or tool offsets.
Keep it simple.

1. Move the origin of the part such that left lower corner is at 0,0,0.
2. Let  the top of the part be Z=0.
3. Read Tutorial #6 in the LC Manual.
4. Rapid, cut start, and cut depth should be in mm's or inches ie, 13.0000 units and not 13,0000 units

Now you should get the Z movements you want and then can play with other settings in both LC and Mach. The code relates
to 0,0,0 so easy to follow the code.

I have similar tables / stages used for my engraving machine. They had steppers and work great, but , so as posted need to find out what
kind of motor's your's have. You may be speed limited due to the tables gearing and axis screw. IE; Mine are limited to about 4 IPM, but, that was not a concern for me as I was after extreme accuracy ( I can't measure the inaccuracy on mine ).

Mach itself dosen't have a "magic wand" to do what you want to do. There are some options you can consider and time management is a decision only you can make for what you want to do. As Hood stated i also believe that having it in CAD gives you manny options.

One consideration is accuracy of reproduction since it makes a big difference in how it's done.

- CopyCat can trace the pattern, large or small, and would provide the gcode. You backplot the gcode and end up with a CAD file.
  You will need to use your machine along with a WebCam ( cheap ......one), but most important is using something like the XBOX 360
  to help make tracing the machine movements easy.

- You could also use photography to get the image and then procede in a number of different ways.

Quantity, size, accuracy, cost, etc all come into play.

BTW, for me at the hobby level, usualy one of a kind, rather small but could be complex,accuracy is very important, and time is not a big factor. :)

I would suggest that you upgrade as you are not going to find much support for ancient versions of Mach.
I would also like to suggest that you don't use Photoshop and learn / start using a vertor based drawing program to create the dxf file.
There are free ones available.
You file contains line fragments to represent the flange and ends up generating a very large gcode file. For kicks, I redrew your flange and generated g code for the flange from LC and then modified that file using the Multipass Wizard for 5 depth passes and all worked well.

Your machine will only go as fast as the max velocity you have set in motor tuning.
Can't speak for Devwing but in LazyCam you must set the feedrate else it will default to  60 IPM in imperial,
not sure what it does in metric mode. So have a look at your gcode's feedrate and compare to your max velocity.
