I haven't looked at you xml but it sounds like you y axis switch may be a little flakey or be picking up noise.  Does it help to set the debounce in general config to 2000, just to test with?

Yes, check all mechanical bits first.  Loose pulleys on shafts, worn keys, etc.  Software will rarely be intermittent, it usually does the same thing over and over no matter if it is good or bad.  A loose set screw, pulley, bolt, keyway can seem to have a mind of its on.

Sounds like you have your direciton backwards.  G-code follows a standard convention and you will want to learn about the right hand rule to set your machine up properly.
