Where did you change the wires at?  In other words, when you changed them what does it prove is good or bad?  Can you swap the wires at a different point to narrow down the possibility's even more?

Also, what OS are you running on the PC.  LC will only post to Mach if running in XP.  Otherwise you will have to save your code then open it from Mach.

Are you sure the home switches aren't repeating or is it skipping steps in the run somewhere creating a cumulative error? 

I would slow the homing speed way down for sure.

Mark, post your xml file and I'll look at it.

It's entirely possible that some of the hardware has given up.  Can you swap your wires going form x or y (don't try this with an axis that has 2 motors) motor to z motor easily?  You could start narrowing it down this way.  You have 2 known good axis and one dude.  By swapping wires you can narrow it down.  Really only a  few things it can be as far as hardware goes.  Motor, drive, power supply for that axis, breakout board, or wires between any of them.

Thats not a backlash spread sheet, it is for steps per unit.  These numbers really should be known if possible.  That is the only way to know for sure you have it right.  You can set your steps per and measure a move using a dial indicator and commanding a move of a set distance from mdi, compare the two and adjust your steps per in motor tuning according but what if it skips steps while testing?  What if there is backlash and you have not allowed for that?  Can you not find the specs for the machine?
