Hello Natefoerg,
Can you possibly be more specific regarding the changes you made on debounce setting. Do you remember if you increased or decreased?
Perhaps you could share the current setting that works for you and type of spindle sensor.
Looking to try threading soon and would really like to avoid the "Chased this problem intermittently for 2 years. drove me nuts" part.

You can also use G52 to offset the axis you want.
G52 X5Y5 creates an offset in the x and y of 5 inches(in inch mode).
You must remember to cancel any such offset with a G52 X0Y0Z0 or call out the new desired offset.

This is probably well known, but I was happy to find it a while back and it has come in handy.

Hello all. Where specifically can I download mach version r3.042.020 ?  This seems to be the most recommended version for current use(with the smoothstepper at least), but I can not seem to find a link to the actual download. I did check the FTP site. If it is there, could someone please paste a direct link?
