Title: dumb question

If you scale down a drawing "mach3" scale do you have to scale down the end mill also.

Yes. ( if you want the contour to be correct without using cutter compensation )
When you use cutter compensation ( G41 ; G42 ) then no, you can use the same endmill.
But the smallest radius in the scaled contour must be greater ( or the same ) as the endmill radius.
This may be a problem when scaling down. A use of a smaller endmill may be necessary
