If you use timing only one slot needs to be wider so mach can use it as the trigger.

You need to post some screen shots of your settings, 4th should move independant of any other axis, you may have it slaved to X by mistake.

Mach4 has to be in 'Disabled Mode' but the option should still be on the menu even if 'Enabled' but it will be greyed out

Check the Ports and Pins, Motor Outputs under the Config menu and see if you have 4 axis configured and if yes then is it A,B or C that is ticked.

Once you know this you can go to MDI and type G00 G91 A90, B90 or C90 and the axis should move.

That Gcode is producing the black line perfectly by the look of it.

place your setting ring at the farthest most point of the machine in X&Y from the home position, this on most machines is the least used place so is the lease worm part of the screw and slides. calibrate there and then edge find there.

If you find the same errors then its time for a new machine.  End of problem.

You could always have the machine laser aligned and mapped but the backlash is your main issue, to maintain the accuracy you think you need your machine mechanics need to be a power of 10 better, so you are not going to get it.

Is the error reversed if you probe the opposite face?

Also is there an error in the other axis?

Have you another gauge ring you can measure, the other may be oval
